Description
This one-month elective Advanced Pharmacy Practice Experience (APPE) provides Pharm.D. students the opportunity to contribute to the care of patients with a variety of cardiovascular thrombotic diseases. The student-pharmacist will engage in patient-centered care, medication therapy and disease management, and collaboration with other health care providers as an integral member of the inter-professional team. The student-pharmacist will deliver patient-centered care following the Pharmacist Patient Care Process (PPCP) model.
